What is a Panic Attack?

A panic attack is a sudden surge of intense fear or discomfort that triggers severe physical reactions when there is no real danger or apparent cause. Unlike general stress, which builds up over time, panic attacks strike abruptly.

The experience can be terrifying; many people believe they are losing control, having a heart attack, or even dying.
 

Signs & Symptoms

Panic attacks can happen anywherewhile driving, at the mall, or even sound asleep. Symptoms typically peak within minutes and may leave you feeling exhausted once they subside.

Physical Symptoms:

  • Rapid, pounding heart rate (palpitations).

  • Chest pain, tightness, or a feeling of smothering.

  • Shortness of breath or hyperventilation.

  • Trembling, shaking, or profuse sweating.

  • Nausea or abdominal cramping.

  • Dizziness, lightheadedness, or feeling faint.

  • Chills or hot flashes.

Psychological Symptoms:

  • A sudden sense of impending doom or danger.

  • Fear of losing control or "going crazy."

  • Fear of death.

  • A feeling of unreality or detachment from your surroundings.
     

Note: Because symptoms of a panic attack can mimic serious health conditions (such as a heart attack), it is important to seek medical attention if you are unsure of the cause.

Many people have just one or two panic attacks in their lifetimes, often due to a specific stressful situation. However, you may be diagnosed with Panic Disorder if:

  1. You have recurrent, unexpected panic attacks.

  2. You spend long periods in constant fear of another attack.

  3. You change your behavior or avoid certain places to prevent an attack.

1. Diagnosis

Your provider will determine if you have panic attacks, panic disorder, or another condition (such as thyroid or heart problems) that resembles anxiety. This may include a physical exam, blood tests, and a psychological evaluation to discuss your history and stressors.

2. Psychotherapy (Talk Therapy)

Psychotherapy is considered an effective first-choice treatment.

  • Cognitive Behavioral Therapy (CBT): This evidence-based therapy helps you learn, through your own experience, that panic symptoms are not dangerous. We help you gradually re-create symptoms in a safe environment to desensitize your body to the physical sensations of fear.
     

3. Medication Management

Medications can be used to reduce symptoms of panic as well as depression.

  • SSRIs (Selective Serotonin Reuptake Inhibitors): Medications like Fluoxetine (Prozac), Paroxetine (Paxil), and Sertraline (Zoloft) are often the first choice due to their safety and effectiveness.

  • SNRIs: Medications like Venlafaxine (Effexor XR) are also FDA-approved for panic treatment.

  • Benzodiazepines: These are sedatives (such as Xanax or Klonopin) that provide rapid relief. We generally prescribe these for short-term use only, as they can be habit-forming.
